  Make an appointment with a lawyer here: http://interwencjaprawna.pl/en/ it's free (EU funded). They helped me write a proper appeal. I had a similar situation - the rejection claimed I hadn't submitted things which I actually had. The lawyer wrote things clearly and officially in an appeal and resubmitted the supposed 'missing' documents. Within 2 weeks, I received a positive decision.
