  Dallas Delilah Velez-Herwat 419-255-7883 ext 169. Ask for Asia, she is the scheduler. Dr Marshall is the Dentist at NHA Cordelia Martin Health Center. You can come with no Insurance and the day of your appointment you can talk to a Specialist named Deb (ext 100) and she will help you with either applying for Insurance or getting on a sliding fee. You can also ask Asia to transfer you to Deb so she can let you know what you need to bring with you to your appintment.
