Personal Reflections Office Policies

Please note, massage therapist does not diagnose illness, diseases, nor prescribe medication or treatments.

1. Office hours are by appointment only. Service fees are subject to change.
2. Payments are expected at the time service is rendered and are only accepted in cash or check. A $20 fee will be charged for returned checks. No post dated checks will be accepted.
3. Any change to an appointment time, including cancellation and/or rescheduling, 24 hours or less before appointment time will result in a fee equal to the FULL AMOUNT of cancelled or rescheduled session fee. Another session will not be allowed until cancellation fee is paid in full. Emergency cancellations are determined at the therapist’s discretion.
4. Reminders of an appointment are a courtesy NOT a requirement. Not receiving a reminder is NOT an excuse to cancel less than 24 hours without paying the full cancellation fee. The full cancellation fee will be due whether a reminder was sent or not. The client made the appointment and will be held responsible for that appointment.
5. Outcall massages follow the same cancellation policy. If client cancels appointment due to emergency, but therapist has left the office and is in route or arrives at destination, there will be a $10 cancellation fee due.
6. Non-emergency cancellations by clients three times or more in a three month period will result in therapist requiring payment at scheduling. If appointment is cancelled at any time, fee will not be refunded.
7. Sessions begin and end at scheduled times. Sessions begun late due to the client arriving late, end at the appointed time and client is charged full amount of scheduled session.
8. Client must provide a health history and update when necessary.
9. Therapist reserves the right to refuse treatment.
10. It is the responsibility of the client to keep the therapist informed of any current medical treatment and provide written consent from the physician, chiropractor,... show more
physical therapist, etc., to the therapist that session may be continued.
11. Therapist will not treat clients with a fever or contagious condition and client is required to reschedule appointment. If client arrives under the influence of alcohol or drugs, client will be rescheduled and must pay in full for appointment.
12. Sexual behavior of any kind (jokes, comments, advances, physical contact, etc) WILL NOT BE TOLERATED. The client will be asked to leave immediately, pay fee in full, and will not be rescheduled or referred to another therapist.
13. Therapist keeps all records and information within the session confidential unless client releases therapist to disclose information or via court subpoena.
