  So...for what it's worth....AZMax did a great job for my oldest daughter in the fall. $2800 out the door cash. My youngest needed the same last month and I accidentally called Dr. Dale Lentz in Tempe and asked their pricing...$1300 less. He's old sc hool and the office is right out of the late 80's (not nearly as pretty as AZMax) and we didn't get tumblers. But he did call me at 9:00 the night of her surgery to check on her and then had a follow-up appt just to check on things, too.
