  I used http://www.insureyourhorse.com/ and selected Equine Insurance Specialists from Muncie, IN. Sadly my horse had a navicular injury after 120 days of coverage. I provided receipts and they paid in full within a week. I'm in LR, AR. It was crazy but they were awesome and we did everything over the internet.
