  I would suggest trying to find a McKenzie certified physical therapist. It is sounding like a problem associated with a mechanical derangement of the lumbar spine. It will typically refer to the hip or you may also have a hip derangement. It would also explain the recurrence. If you go to www.mckenziemdt.org , you can locate a provider near you.
