  I am 27 and wasn't able to go to the dentist for years because I couldn't afford it. I recently found [InterCare](http://www.intercare.org/holland) in Holland. They charge based on income. I am a student working part time, for me there is a flat $40 co-pay for most dental work (extractions, fillings, cleanings, exams, etc) and a $20 co-pay for medical. I haven't used been in for a medical check up but I quite recommend them. I will be having my four wisdom teeth pulled next month and will be charged $40. Of course what scale you pay depends on your situation, but I think this place is a financial god-send.
