  I know this isn't exactly what OP asked, but I just joined a kind-of fancy gym/spa on St Denis and Duluth called Le St Jude, and the monthly membership fee includes a 1 hour appointment with a kinesiologist, in which they create and adjust a custom exercise routine for you. They even print it out on a fancy little worksheet, so you can check it off as you go. I was previously interested in getting a personal trainer, but now I realize that this is all I was really missing. http://lesaintjude.ca/
