  I completely support your decision. I have a healthy 12 year old who has not been vaccinated. He did not get chicken pox when all his vaccinated friends did. When his friends who get their halt flu shot get the flu, he stays healthy. He rarely gets a cold. The exemption form is simple to get. Drive to the vital stats office, wait in line, and sign the paperwork. Try Dr. Joseph Ciccarello. 813-621-3180. He is a great doctor. Not sure about the medicaid, but his office manager Tammy will be very helpful.

  Well, I can't guarantee his schedule for appointments, but we like Dr. Ciccarello. He's on 301 South of 60. His office number is (813) 621-3180. He will accept you with no insurance. I think he charges around $35/visit.
