  I'm fond of the design at http://www.acurator.com/ I've seen some photographers use same. the only thing I might add is that I've seen a few panels and roundtable discussions by art buyers and the one thing that they ALL agreed on was to have your name, phone and email on every page. They hate clicking around and looking for that info.
