  Dr. Port in Holland for sure! My son is orally defensive (although less than he used to be). I had a friend recommend Dr. Port in Holland... she specializes in kids with special needs. We went to see her and she was amazing with my son! He used to go every 3 months to get him used to the dentist (and she doesn't charge for every other visit) but she worked and worked with him and after 1-2 years he is now going every 6 months but brings his own toothbrush which the dentist uses to clean his teeth.
