  Try calling Pacific Wireless... They are the ones that found the best outfit for my area (which also happens to be DISH) 208-457-8077 They will know what works in your area and they are awesome.

  I don't know if you could apply from Canada. If you give them an address in US, then maybe you could. I don't know. I'm still in US. I had a regular Nationwide plan with AT&T. After my PR application was approved, I upgraded to North America plan. For existing AT&T customers it was only $10 extra. Even after Cingular purchased AT&T, I decided to keep this plan. I just tried to find similar plan on cingular.com site. It seems like they don't have it anymore. Actually my old AT&T plan is better than new Cingular plans. You could check Verizon's "North America's Choice" plan, but it is more expensive than old AT&T plans.
