  Sent in: Cheeze please help me by posting this. My son needs a TB Test to be registered for school, but his doctor ran out of it, and they are not getting any soon. Does anyone know where else I can get one. I have called other pediatric offices, but the only way they'll give him one is if I switch doctors, and I don't want to do that. I just need one TB test.   Instructions for requesting the official exemption affidavit that must be signed by parents/guardians choosing the exemption for reasons of conscience, including a religious belief, can be found at www.ImmunizeTexas.com. Original Exemption Affidavit must be completed and submitted to the school or child-care facility.
