  618-259-5563. This is where I take one of my daughter's she has all kids insurance card too. I think it's call Affordable Dentistry. Or you can try SIHF in Bethalto. 618-259-1419 call and ask for pedi dentist. They take All kids if they don't have any secondary Insurance from somewhere elese. Good Luck
