  Kelly we live in Tiffin and use Dr. Christian Meade. He has been awesome with our son and has never brought up vaccinations after our initial well baby visit. I never had to sign any incriminating forms and he is also good about laying off the antibiotics. His office is located at 433 W. Market St. Tiffin and the number is 419-455-8150
