  08-641-8767 Beersheva office is those new buildings on Rager between the Omanuyot haBama building and Bialik. 08-972-9410 Modiin office close to entry to city - doable if you have to (I have had to). He's very good. With parents and with the children and of course with the teeth. He himself comes once a week to Beersheva (Mondays I think). Other days he has a partner here - Galit - who is also very nice. Personally we relied on his judgement more but we may have been biased. Ask for him at least in the beginning if you can (IMHO)
