  Not sure if you have had sucess yet, but I picked up a. 'Routine Fee Schedule' , the Foundry Dental. Cntr., 1700 6 th Ave. N, Bessemer. 205-434-2031. Not free, but greatly reduced. This was on the table at the B'ham Vet Cntr in Hoover. 205-212-3122
