  http://www.dbclinic.cz/en/hlavni-stranka/ ask for Aleksandra. She's Polish, so she definitely knows Eng. I have to recommend her a lot, she helped us from big nightmare with other Czech doctors. And prices are good (also VZP). Her boss is also good
