  I don't know about your price range or their days...but, I highly recommend Dr. James (775) 782-4106 (He, singlehanded, cured my dental phobia...he is great...office staff is great...I have crappy insurance, but, the office maps out a treatment plan and shows you all the costs up front, ahead of time, so you can plan and prepare in advance.)
