I'd recommend an architect if you feel there are many options in how you can extend. We're just finishing an extension and can highly recommend Chris Todd architect http://www.christoddarchitecture.co.uk/ who gave us so many ideas to consider and were very clever ways of using the space. We're now using M.J Salmon & Son Ltd (Matt Salmon) who we have been very impressed with as he is project managing it and keeps in touch with us constantly by email and phone.
