Our nonprofit used Lookout Threads https://www.facebook.com/lookoutthreads/, they turned out good and the quality of the shirt itself feels great.

I work at a local urban high school and we have a lab that can design and print t-shirts. We can do one custom t-shirt or a large order of shirts. We also offer laser engraving, 3D printing, and other fabrication tools.
