Dr. John Nelson. He lives in Walnut and his office is on the corner of Fairway/ Colima behind McDonalds

I go to Costco and they do take some insurance. Dr. Nelson is really good but a little expensive. Bring up his daughter Amy and prepare to listen:-)!! He's a frequenter of N. Y. Pizza. Good luck:-)!!
