Not sure about Dr Crosen...he does all the pain management at Coquille Valley Hospital from what I gather. Maybe call the Hospital to find out for sure? And if he's taking new patients? They are very nicely staffed.

Your MD will and can do it... It just has to be your regular doctor you can't go see a doctor just to get your card signed. They have a doctor every month at the Red Lion I don't know about any prices at or with that with that. I know a doctor at Bay Clinic gave the one to my girlfriend
