Pam Piazza works through Hawthorne but will do at home training as well. Very good trainer, I have worked with her and she is great with all sorts of issues. 618-288-3971 for training info.

I highly recommend Bob at k9 packleader.com. He's tough, not cheap, but worth every penny. He comes to your house and ...the only way I can explain it is to say he's like a dog whisperer. My rescue dog had a lot of reckless anxiety (which sounds like what your dog is suffering from) and within minutes my dog was obeying orders and now we live a very calm and happy life. I'm also a volunteer for Stray Rescue, and I know Randy Grim has called him a time or two for help dealing with a couple of troublesome rescue dogs. Tell him Erin on Neosho St said hi!
