Your best bet is to go into a local Wells Fargo and talk to a business banker. They have all the training you need to help you start a business, and are happy to do it for free. They should even have a stack of the forms you need for things like DBA accounts and TIN's.
It does not matter if you bank with them, or plan on banking with them. In fact, I would make a point to go into two different Wells Fargo locations and talk to business bankers. You never know who listened in training and who didn't. Then go to 2-3 other banks and ask the same questions.
You will get a very in depth education on banking and setting up a business from very impartial people (of course they want you to bank with them, but the rest is completely free, without strings). You might also find a banker you really like. Having someone like that local is usually far more important than finding the 'best' banker, and they tend to be connected (think facebook/linkedin) with other local like minded professionals, like lawyers and accountants.
