Oh also if you'd like to find one near your neighborhood, you can call Tokyo medical info. The number is 03 5285 8181. Just specify that you want an English speaking dr. They'll ask you which station would you like it nearby. :-)

Dr. King in the King clinic on the intersection of Meiji-dori and Omotesando. Bilingual and takes national health insurance. We got very speedy vaccinations before our trips and he has good hours. It's in the building next to Condomania and there's a bug sign on the 3 rd Floor window that says "King Clinic". It's on the TUC side of Omotesando...just go down to the intersection and look up!

Following up on Heather's info above, if I recall correctly, the doctor's name is Dr. Leo King. I have never seen him before, but he is highly recommended by my hubby's Japanese colleague.

Jamee, we use Dr. Lomax at Tokyo Medical Surgical. He is easy to work with and will do shots according to the US shot schedule. The clinic is right by Tokyo Tower.
