If he's a certain type of ADHD stimulants may not work, in fact worsen so it all depends on the type. Some benefit from caffeine, some not. Some from Meds, some not, us included. I have been through this with my 7 year old for the past 1-2 years. He cannot control his impulsivity and focus to get much done. It sounds like you may have but have you read the article by Dr. Amen about the 7 types of ADHD? We are currently noticing a peak in increased sensitivity in auditory and sensory processing and I'm waiting for a neurological Assesment to get further answers and see if he's on the spectrum. Vigorous exercise and "heavy work" and earmuffs seen to help him lots. He has only been diagnosed with ADHD and odd but his case is classic for those on the spectrum so we will see. Try to ask what's bugging him, more often than not, there's something we don't pick up:) id also like to try Accupuncture and Neurofeedback therapy. Hope this helps! You're not alone!

We are in the absolute same situation as you but my son is 8 and we just finished 2.5 years of med struggles. Started Vayarin (Omega 3, exercise, closely monitored diet, counseling once a week and also just started Epsom salt baths. I too am very interested in neuro feedback.
