I had a pup shipped to me from Germany a while back and the breeder used a company G.K. Airfreight Service (www.petshipping.com). Not sure if they are only in Germany or not, but they did a wonderful job.

Recently had occasion to ask a friend about this on another FB page. Here is her response: My travel agent is great she flies a lot of people with show dogs to Europe. Dana Huckestein at Lewis Travel in Fresno.If the dog isn't small enough to fly in cabin, she'll have a hard time getting there during this time of year..... Dana's phone is 559 431-1800. I'd start there if the person is going with the dog. If not you might start with Luftansa (I know they ship cargo, possibly at this time of year).
