Tokuda hospital does the full package, with an English-ish speaking nurse to help you from one department to the next. Takes about 3 hours and 250 leva, depending on the number of tests / certificates you need. http://www.tokudabolnica.bg/

Step 1: Go with a Bulgarian speaking friend to the Perva Gradska Bolnitsa on Patriarx Evtimi Step 2: Take the entrance to the right and walk up directly to the 2nd floor. If a guard ask s what you are looking for say "Imame sreshta s lichen lekar (GP) na gore" and continue. The point is not to pass by the Kassa where they will ask and give you papers for EU refund. It s a complete waste of time for 20 or 30 leva. Just pay "Na broi" to the GP but make sure you have 10 and 20 bank notes in your pocket. Doctors are allowed to receive directly money and it avoids the papermill downstairs that will drive you crazy. Step 3: Check the doors and find where there is a GP on duty. Step 4: Wait your turn and weh the doctor asks if you have documents tell him "Niama nujda shte platia na broi" Last year it was was 20 for a GP and 30 for a specialist Step 5: Give him the document you want to be signed. He/she will ask you a few questions and if you don t show weird signs you should get the signature and stamp (Petchat) I guess you can proceed this way in any public hospital. Just make sure you find the right door and avoid guards that lead you to the Kasa. This will not work in Tokuda because they are organised at the counter. In Tokuda you wont need a Bulgarian speaking friend either I presume. Thorax Medical Center should also be OK. Maybe easier than the one I mentioned because it s smaller and maybe a bit more expensive meaning not overcrowded.

You can find an emergency eye doctor at https://www.facebook.com/137832933029369 Умбал Царица Йоанна - Исул, that's what I know from experience and also, a while ago that was the only place that you could find one. No idea if the one on shift will speak english. Also, as Vladimir Vassilev said, you can call 112 and ask.

Dr. Ivan Tenev or Dr. Daskalov from Pentagram clinic
This is almost, but not quite, one of those "why don't you use the Search facility" questions ;)
Briefly, has anyone had any experience of eye surgery here in Sofia? ...

Kostas Jarvis, about a year or two ago my best man who's British, btw, and has seen the world and knows what he wants and how to get it, got his eyes one after each other operated on in the Zora clinic, an ophthalmological clinic occupying a floor in Clinic nr. 7 (near the railway station). He's been a new man ever since - got rid of the glasses and has no words to praise the staff there. I can find out names of doctors, but we were all so impressed with his positive transformation. Here's a link, good luck whatever you choose! www.bolnica-zora.com
