I went through the process about 18 months ago. I ended up going with a hosted provider for all of our SfB and Enterprise Voice. You could contact Enabling Tech, they would offer services you are looking for. I have worked with them before. They are very good at what they do and are MS Gold Partner. I know that AudioCodes has a product that is an all in one piece of hardware. I'm not sure if it can be leased though. If you want to consider a hosted provider, I'd recommend CallTower. If you're just looking for a SIP provider for your on premise SFB, Intelepeer is one I've worked with quite a bit. Just make sure whoever you go with is a Microsoft certified provider.

Having an SBC adds a little cost and complexity, but it gives you a lot of control over your voice traffic. And there are tons and tons of Microsoft endorsed systems integrators out there who can help you purchase, install, and configure all of this.
