I recommend Julie Pearce, but I'm not sure any local therapists take insurance for payment. You may have to pay then get reimbursed by your insurance. Her # is 541-264-1159. BTW - I have suffered from the same kind of pain, and we have been working on it. It takes a few sessions. I've dealt with it for many years and massage and a bit of Trameel are the only things that have worked for me.

Nicole Mccauley at central coast chiropractic in Depoe bay. She uses the actuator and no manipulation. A bit of a drive but very low rates. She's in Africa right now but will be back next week I think.
