I recommend Damian clinic on Foksal. Most staff speaks English, and the doctors are good. A regular visit is 130 zl.

Luxmed and Medicover are the two main private health organizations in Poland. I use Medicover and it's fantastic. Lots of english speaking doctors and an incredible online service. Damian medical centre on Foksal is also nice, though I haven't used it, just been inside. They are somehow affiliated with Medicover, but I'm not sure exactly what the connection is. Hope that helps.
