Just copy paste from pit11 to pit37 ( I assume you need 37 th) and there you go. They will send it for you to the tax office and in a few weeks you will get your tax return in case of any...

I can't recommend Antonello Galasso more for doing your USA taxes. He's been living in Poland for years but has a US-based accounting business, so is familiar enough with the Polish side that he can be of help. He does not do your Polish taxes, though.
