Jeff, there's a firm called ComforCare that will do transport to & from procedures for $55. I've used them several times. They're great. They use medically trained people to drive you, and they'll sign in as your "person," so whatever provider you use is good with them. (Unlike an actual taxi.) Their number is 718-9393.
