I can do the formatting for you. I like the practice. I format all my ebooks in HTML/XML from scratch and use InDesign for print formatting. Send me a message and I'll send you my email.

There are a bunch of people who would be willing to format for whatever format you want including print. I do the same thing, but it looks like /u/Nepharid beat me to the comment about it. :) I happen to use DocBook to create my output; as long as you have pretty plain formatting, it is easy. In general, it depends on how much fancy formatting you want (putting a fancy image behind the chapter titles in print takes a bit more effort).
