  I think Visionworks may be a Humana provider-either in DeLand WD plaza at 17-92 and Orange Camp, (386) 736-3579 or in Seminole Mall 2 nd floor near Dillard's. (407) 328-0790.

  Try one of these providers if Visionworks is not participating. My mother has Humana and I recall her mentioning Visionworks. Better yet, call Humana number on your membership card and they'll steer you in the right direction.
