I don't care if it's a man or a woman or a private practice vs corporate place, so long as they are gentle, and willing to put me to sleep for teeth extractions. None of that laughing gas nonsense, because it doesn't work on me. Who would you recommend?

I went to Dr. Paul Matthew in Salem. http://www.drpaulmathew.com/ Would highly recommend. He's knowledgeable about tech, very friendly, has a great staff, and most importantly is trained in sedation dentistry. You need to have someone to ferry you to and from the appointments, but you'll not remember the bit inbetween. Good luck!
