  I know that Pen Air & Harvesters has them. Pen Air charges like $5 i think. Another lady is Ida's Forms...She drew up all of my divorce docs for me for about $150, so she also does notaries. not sure of the price...(850) 434-7524 is her #, you have to call and make an appt.
