  I get my mom gift certificates to [Azario](http://www.azariosalon.com/) occasionally. She seems to really like them. They do all of what you'd mentioned, too. [Here are their package prices](http://www.azariosalon.com/packages/index.html). Edit: It's up Starkey Road, on the opposite side of 419 than Tanglewood. (If you're coming from Tanglewood area, you'd take a left onto Starkey, if you're coming from Salem area, you'd take a right.) It's a little ways up Starkey on the left, so keep an eye out, if you do go there, it looks like a house with a gravel parking lot in front (though it does have a sign as well), and you might miss it if you're looking for a more office-like building.
