  Hey Tammi Coronado try this site http://fmaaa.org/serviceproviders.htm and download the pdf at the left under Resource Guide. More than 15 years ago I had gotten my father signed up for a very affordable service. Had to take him to a tiny office on Blackstone at the curb in front of the mall at Shields for a photo ID. They even came out to the car to take the photo so I didn't have to get him out and in wheelchair. Anyway, a call or two to find out what is required should produce help, I think. Good luck.
