For menswear we swear by John the Tailor in Framingham on Rt 126. He is amazing; not inexpensive, but he knows his stuff. For dress alteration specifically, I have had great luck with Bridal Fashions & Alterations in Wayland; the tailor there, Sienna, is fantastic. She altered Sophie's bat mitzvah dress perfectly, quickly, and not too expensively.
