Deanna Burt, I'm not sure. Please check out their website above. Lisa, Just an fyi, it's a discount plan, not insurance. The doctors in their network offer a reduced price for different procedures. You must go to the doctors in their network, but I've been satisfied both times I've used it.Coast and Aspen Dental are providers in Lakeland, unless that's changed within the last couple mos.
