It sounds like what you're looking for is an integrated/coordinated care system that provides a network of primary, specialty care, and other services. If that's the case, your best options would either be reaching out to Centura (which owns Parker Adventist) or getting on board with Kaiser Permanente. Those are the two largest systems with clinics in the area. I've been in healthcare and worked on complex cases for a number of years so you're welcome to PM me with any questions.

I live in Miami, FL. Theres a group called Leon Medical Center. It's what she has here. It is both the insurance and the clinic. They offer her transportation and all specialties. And they're affiliated with certain hospitals. They basically provide everything she may need in one network. Thank you for your help. I'll look into it.

Parker Adventist may or may not do everything you want... quite frankly the whole point of living in a city (ie closer in to Denver) is access to those things and you may not find what you want in a more sparsely populated area.

It does depend on her coverage and frankly, her income. Where do you live/what group does those things? Kaiser is great for having most groups in the same complex. But very few groups offer transport. Innovage is perhaps something you should look in to. Access-a-ride is offered by RTD but you have to qualify based on disability. Is she enrolled in the PACE / innovage program? It will transfer counties. Otherwise there's a large chance she's eligible for long term care Medicaid insurance.
