John..regarding the ophthalmologist: it really depends on the condition.. ophthalmologists sub-specialize; for example, Dr Dietze is an excellent cornea/cataract surgeon but others specialize in the retina/glaucoma/etc. If you see an optometrist first, they can guide you to the correct surgeon.

My brother is Dr. Ramos at Complete Family Eyecare. He's amazing with kids, extremely patient. Located in Kohl's shopping center. He's been President of the Optometry Society for years. And, was the first Board Certified Optometrist in CC & might be the only one still. He's truly brilliant!
