I went to: Michael's Jewelry Center Inc on Centerville road. I didn't get anything as interesting as what you're describing though. I went for a ruby in platinum and he was very helpful. Full insurance documentation, JIS certified. Definitely worth a stop in. Gives free cleanings and inspections. Very easy on resizing.

Elizabeth Diamond Company in Centerville is supposed to have lots of customization options. No idea on the price though.

It's a bit of a jaunt, but my parents and grandparents both got theirs done at Hug Jewelers down in the Cincinnati area. My mom got a pretty customized piece and I've had repairs done on the set my grandmother left me since she passed, they do good work.

We had a great experience with ours from ValerieKStudio on etsy. Not local but might be a good bet. She worked extremley well with my fiancee designing a good portion. I'm sure there are some other reputable Etsy shops that are just as great as well.

If you're willing to drive a little, definitely check out Chelsea with Honey Designs. She's just over the boarder into Covington, KY, but she's amazing! She will fully involve you in the creation of your ring and stick to whatever budget you'd like. You can design the band, types/number of prongs, and choose out your stone. I made my engagement ring with her. I was thinking morganite for the pink tone and decided on a pink sapphire. We had an initial meeting to talk about the process and my ideas. She then sent out a few design options and we met again to fine tune what I liked. We then had a third meeting to pick out the stone. My sister also made a band with Chelsea to match her vintage style engagement ring. Honestly, she is so friendly and honest throughout it all, I can't speak more highly of her.
As far as aftercare, she will include a 1 year warranty with the ring and she recommends I have the prongs/setting checked yearly.
