Steve at Old School Tattoo in Bellingham has done great work for several of my friends. One is a full sleeve based on a chapter of "Everything is Illuminated" -- she told him the parts of the story that were most meaningful to her, and he came up with an amazing design. I recommend sending him your ideas so he can work up some options before going in so you don't have to pick random flash from a book.
