I just looked it up because you peaked my interest on this subject again since my husband has a business. Pa.gov has all the forms. $125 for the domestic llc another $70 for your fictitious name registry and an additional form that has no fee but needs to be included when mailed. So $195 total. Forms can be downloaded as pdf and filled in on computer (typed) then printed and mailed out. Its that simple. Good luck. We will be applying for our LLC also.

Make sure the POA is the type that gives them medical control. If it is you may have to challenge for guardianship. Ultimately it is up to the medical providers to initiate hospice with the family in agreement. My mother has been under the care of Excela Hospice and staying at Greater Pittsburgh Rehab and Nursing Center for the past few weeks. They have counselors that can answer any questions.
