Moniot Amal is the company that runs them. Their phone is 04-866-2324. I don't know how far in advance you can book them, but you can just check with them. I personally in general dislike sheirut services, but theirs to the airport was actually quite good when I took them.. it was last year on a shabbat, and it cost me 87 shekels. One thing, though, make sure to keep on checking with them if they say they don't have a sheirut leaving when you want it to. They may open new ones if enough people want to go at a specific hour, or or course, if someone cancels you can get their spot.:)
