If she is in hospital, they should be contacting social services to give the family help and advice. They will need to means test her for financial assets , ie savings and property to sell . Does she need nursing or residential placement, the cost per week is different. Have they considered a live in carer , might work out cheaper if she doesn't want to leave her home ? Definitely get the continuing health care assessment done . Look on websites for homes and check out their CQC reports .www.carehome.co.uk , search by county. Savings are £22.250 .

You need to make sure the ward staff refer the case to Social Services before discharge. They will wait until almost medically fit before doing this but you need to ensure if it happens. SS will carry out an assessment of care needs and also of financial circumstances to assess whether a contribution or the full cost of the proposed care package is needed. HTH
