Wuollet's bakery is my go-to for custom cakes.

Sweet Retreat is great if you want something fancy. Lunds/Byerly's have excellent cakes if you want something basic.

I actually think Lunds has fantastic cakes. They should be able to do custom. Wuollet has never let me down either, and they routinely have gorupons... so get it half price!

This might be too fancy, but we got our wedding cake at Queen of Cakes in Edina. They can do lots of different things and the cake was delicious. We were very pleased with our experience there.

Yum Bakery in St. Louis Park is amazing. A Baker's Wife makes a really good chocolate peanut butter cake, and I have a soft spot for the CostCo cake with the custard filling (we always have one for office birthdays, retirements, etc.)
